The first page of the application form is to be completed by you. The second page, which includes the diagnosis and justification for the rehabilitation (improved resilience, reduced fatigue, etc.), must be completed by your doctor. Your doctor should indicate “outpatient rehabilitation” and “oncology” and propose a location (e.g., Klinik Pirawarth in Vienna). The completed form must then be sent to your insurance provider (for working individuals, to the PVA), which will issue the approval.
